Two weeks to go, and still no passport.
We called the state dept. passport "hotline" (1-877-487-2778) but they didn't want to talk with us unless it was two weeks before traveling. Since it was sixteen days they asked us to call back before submitting the request to be expedited.

From what we understood, the request has to be made, someone in the passport agency needs to intercept the passport before it being mailed out and mail it next day air. If that doesn't work, we're going to have to hoof it to Philadelphia (in our case), 2 hours away to get it personally.
Exactly two weeks before our trip, after making sure the passport didn't arrive in that day's mail, we called up the passport office. They took down our information and we got a solid promise to "try".
Try they did, and we got our passport after a few nerve wrecking days (overall it was two days, but there was a weekend somewhere in there) ... but no birth certificate (we had to send the original) which arrived several days after that.
Our tax money at work finally being put to good use by competent people.
