I'd like to visit Isla de Ometepe, Nicaragua, then proceed to San Carlos by boat, then take a boat to the Caribbean Sea (via San Juan river), and finally continue my trip southwards all the way to Bocas del Toro, Panama.

According to LP there is a "litttle used and sketchy" crossing mainly used by tour groups from san juan de nicaragua to barra de colorado (CR), you canhire a private boat for $140. And from barra de colorado you need to get a flight to San Jose, and the customs might be quite intruiged as to what you're doing there
